Hospice Admissions

Hospice admissions are coordinated by a registered nurse who is available to answer questions about Hospice care, and to assist with admission to the program. This service is provided free of charge through the Hospice office. Consultation with the patient's physician is part of the admission process.

Who Makes Up the Hospice Care Team?

The patient's physician is the best source of information and guidance in making the decision to select Hospice care. Hospital discharge planners, nurses, clergy, social workers and family members also make referrals for Hospice care.
