Patient Portal Instructions

St. Charles Health System is offering a secure website which will allow convenient 24-hour access to your personal health information from your home, office or anywhere there is internet access.

Patient Portal

We at St. Charles Health System believe that all patients should have easy, instant access to their health information.

Now, through the use of a secure, online portal, you will be able to access your health information, including allergies, procedures, lab and radiology results, vitals and more—all in one place.

All you need to activate this service is a valid email address.

We want to protect your privacy while also making sure that your care team can access your personal health record.

Our Patient Portal can help you and your family or other caregivers access your personal health record online.

Using the system

STEP 1: Click on the orange button at the bottom of this page to access the patient portal.

STEP 2: Click on the "Register" tab at the top of the screen.

STEP 3: Fill out the form using your current information.

STEP 4: Once you've completed registration, log in to your account and click on "Providers" at the top of the page.

STEP 5: Click on "Add Provider or Facility for {your name}" - see screenshot:



STEP 6:
To add the hospital: When the search page opens, enter "St. Charles Health System" and click on the "Search" button. You will see St. Charles Health System (Repository) show up in the search results. Click on the "Select" button next to this listing. You will be redirected to a page with a list of your providers. This message will display at the top:

To add a primary care provider or specialist: When the search page opens, enter the name and zip code of the provider and click on the "Search" button. If more than one location appears in the search results for your provider, click on the "Select" button next to the listing where you visit your provider. You will be redirected to a page with a list of all your providers. This message will display at the top:

Access the Portal



How to request access to relatives' records

Spouses

Each spouse needs his or her own separate account, unless the patient’s situation is one which qualifies under CAREGIVERS FOR ADULTS OVER 18 YEARS.

Caregivers for adults over 18 years

Contact our Health Information Management Department (541-706-7784, option 1) and they can help you determine what type of form you need to complete. Access to another adult’s record requires that the patient signs a Release of Information form, or that you have a Medical Power of Attorney for the patient or other legal authorization allowing you to make medical decisions for the patient.

Once you have completed the appropriate form, a copy will be entered into the patient’s legal medical record. After that step, you should complete the process described on the back in ACCESS TO RELATIVE RECORD.

Parents with children under age 18

In order to comply with applicable privacy laws and regulations, it is St. Charles’ policy that only those patients who are 18 years of age or older may access their information in the Patient Portal.

Accordingly, we do not allow access to a dependent Patient Portal record for patients that are under age 18 without prior approval.

In some situations, parents or guardians of patients under the age of 18 will find Patient Portal access for that dependent necessary to help manage a disability, chronic illness, or other condition, which may require, for instance, frequent labs or other monitoring. In those instances, Patient Portal access may be granted to parents or guardians of children under age 18. If you believe that your child’s situation requires access to the Patient Portal record of dependent, please obtain the appropriate request form during registration or contact the Health Information Management Department (541-706-7784, option 1).

Access to relative record

Register for your own patient portal account by either requesting an invitation at portaladmin@stcharleshealthcare.org or by going to stcharleshealthcare.org/patientportal and following the instructions. Add your relative as a family member.

After you have added your relative, select their record and add a provider.



After you have selected the provider, a request will be sent to that practice. Once it has been accepted, you will have access to your relative’s record.
