Testing locations and scheduling

Below are COVID-19 specimen collection locations, beginning Monday, Aug. 3, for both St. Charles established patients (patients with a St. Charles primary care provider) and non-St. Charles established patients.

St. Charles Health System is requesting all COVID-19 collections on pre-surgery and pre-procedural patients to be collected seven days prior to their surgery or procedure due to the delay in testing at University of Washington.

Requirements prior to going to a collection site:

  1. Do not use nasal spray or nasal salve 24 hours prior to collection
  2. Please blow nose at home, washing hands afterward

If a patient has an existing COVID-19 lab order from a provider, they should present to one of the six collection sites below - no appointment needed:

Non-St. Charles providers: If you have access, please place the order in St. Charles Health System Epic. Otherwise, fax the order to 541-706-6365 or send a paper order with the patient.

St. Charles Immediate Care Clinics are able to evaluate patients and provide COVID-19 specimen collection, if warranted, at the following locations - no appointment needed:

Testing Criteria

Any outpatient may be tested who has

  • Fever (100.4 F or 38.0 C) in the past 24 hours OR Cough OR dyspnea (e.g. shortness of breath, particularly with chest pain)


  • At least two of the following:
    • Chills
    • Repeated shaking with chills
    • Muscle pain
    • Headache
    • Sore throat
    • New loss of taste or smell


  • Whose symptoms are sufficiently severe to require medical attention AND meet one or more of the following criteria:
    • Any patients with worsening symptoms (e.g., a repeat visit to the ED or office or no improvement in fever, cough, or dyspnea in the past 48⁰)
    • 60 years of age or older
    • Age less than 12 months (NOTE: this population may have hypothermia, so a fever is not required)
    • Any child (age < 12 y.o.) with household contact at high risk of severe COVID-19 illness (age over 60, hypertension, diabetes, cardiovascular disease, lung disease, immunocompromising conditions or pregnancy)
    • Patients with underlying medical conditions, including but not limited to hypertension, diabetes, cardiovascular disease, lung disease, and immunocompromising conditions, including cancer 
    • Pregnancy
    • Patients who had contact with a patient with a COVID test pending or with confirmed COVID 19 infection
    • Health care and Public Safety workers (including scribes and EMTs, Long-term care facility or Assisted Living Facility employees, first responders including police and fire personnel, and any worker in a vital infra-structure role, such as an employee of an electrical, gas or other utility, water or sewer treatment facility).
