St. Charles Home Health provides skilled professionals to help patients recover safely and rehabilitate from injury or illness in their own residence. 

Referral required

  • Providers can submit referrals via Epic, fax to 541-706-4996.
  • Seeks to admit within 48 hours.
  • Patients must have seen a primary care provider (PCP) in the last 90 days and be homebound. (A patient is considered homebound if leaving their home requires a considerable effort due to an illness or injury, often needing assistive devices (walker, cane, wheelchair) or another person's help, or if it's medically unsafe. They can leave for medical appointments, church or haircuts.)

Certified Home Health Aides

  • Bathing, grooming and dressing

  • Patient transfers (bed to chair, etc.)
