1. State indication for transfer - usually not a diagnosis but a resource need (i.e. bed availability, procedure capability, subspecialist)
2. Concise clinical summary
3. Accept patient as soon as it is clear that transfer is appropriate
4. Clarifying questions - only pertinent to immediate care needs
5. Any necessary interventions before transfer?
6. Agree on mode of transport
7. Transfer center or accepting provider gives summary statement: Patient accepted by ______ doctor to go to ______ unit by ______ transport.
8. GOAL IS physician to physician conversation of < 6 minutes
